Aircraft: Charles Keuthan ZEPHYR II, registration: N52958
Injuries: 1 Serious.
On March 31, 1997, about 1730 eastern standard time, a homebuilt
Zephyr II, N52958, registered to a private individual, experienced a hard landing during a
forced landing at the Flagler County Airport, Bunnell, Florida. Visual meteorological
conditions prevailed at the time and no flight plan was filed for the 14 CFR Part 91
personal flight. The airplane was substantially damaged and the private-rated pilot, the
sole occupant sustained serious injuries. The flight originated about 5 minutes earlier
from the Flagler County Airport. The pilot stated that during the upwind leg after takeoff
about 300 feet above ground level (agl), the engine experienced an rpm drop from 6,200 to
between 2,500 and 3,000. He then initiated a turn downwind and then again turned upwind
and when the flight was about 75 feet agl, he encountered loss of elevator control. The
airplane then descended nose low and impacted the ground adjacent to the intended
touchdown location.
Accident occurred APR-08-97 at WINTER HAVEN, FL
Aircraft: Basinski KOLB MK3, registration: N128BB
Injuries: 1 Fatal.
On April 8, 1997, about 1200 eastern daylight time, a Basinski,
Kolb MK3, N128BB, registered to Lakeland Ultralight Inc., operating as a 14 CFR Part 91
personal flight, crashed while maneuvering in the vicinity of Winter Haven's Gilbert
Field, Winter Haven, Florida. Visual meteorological conditions prevailed and no flight
plan was filed. The airplane was destroyed. The commercial pilot was fatally injured. The
flight originated from Lake Jessie abeam Browns Seaplane Base, Winter Haven, Florida,
about 1 minute before the accident. Witnesses located at Browns Seaplane Base, and boaters
located on Lake Jessie, at Winter Haven, observed the airplane making touch-and-go
landings to the lake. The airplane took off to the north and turned to the west. Blue and
white colored smoke was observed coming from the vicinity of the engine. The airplane
appeared to lose forward airspeed while in a climb at about 100 to 200 feet. The left wing
dropped down between 45 to 90 degrees, the airplane entered a spin to the left, colliding
with a house trailer and the ground.
Accident occurred MAR-23-97 at PENN VALLEY, CA
Aircraft: Zenair 601, registration: N4073T
Injuries: 1 Fatal.
On March 23, 1997, at 0849 hours Pacific standard time, an
experimental, amateur built, Zenair 601, N4073T, impacted terrain in a pasture during an
off-airport emergency landing near Penn Valley, California. The aircraft was destroyed and
the private pilot was fatally injured. Visual meteorological conditions prevailed for the
personal flight which departed from Nevada County Airport at Grass Valley, California,
about 0830. A witness who lives about 1 mile from the accident site reported that the
aircraft's engine was "cutting out" as it flew over his location. Another
witness, inside his home near the accident site, did not hear the engine at all; he
reported hearing only a "whoosh" and a "thump".
The aircraft was engaged on a local flight in the area of the
Lady Bower Dams. Shortly before reaching the dams and at a height of 4,000 feet, the
engine stopped. The area around the dams was of rugged terrain with very few areas
suitable for landing a light aircraft. The pilot chose a field but when committed to the
forced landing saw that the field was on a steep upward slope. During the landing the
aircraft's landing gear collapsed.
Initial examination of the engine indicated that it had seized,
with no other external indications of mechanical failure. Subsequent strip examination of
the engine by the pilot revealed that there had been a major failure of the rear cylinder
'big end' bearing, but further examination found no fault within the lubrication oil
injection system.
Similar seizures of Rotax 582 engines have occurred and three
earlier AAIB Bulletins (i.e. 12/95, page 55, 10/96, page 29 and 5/97, page 63) which
reported on engine seizures on a Renegade Spirit, a Kolb Twinstar and a Avid Speed Wing,
drew attention to this problem. Although no statistics appear to be available, the Popular
Flying Association advised that there had been a history of big-end failures on Rotax 582
engines, due to worn bearings. This was particularly the case for engines installed in
heavier aeroplanes and those used in the training role which use extended running at high
power. There were also indications that long periods without use could be detrimental.
'Drying' of the roller type bearings may occur and residual products of combustion, which
can be significantly acidic, can accelerate corrosion of the bearing materials. If
corrosion occurs, this preconditions the bearings to wear rapidly when used subsequently,
although it may then take several flights for the bearings to deteriorate to the point of
failure.
The largest service centre for Rotax engines in the UK has
devised an instrument, the 'Cyclone Conrod Bearing Clearance Tester', for testing the
combined big/little-end bearing clearances, which has apparently proved extremely
effective in preventing such failures.
The Bearing Clearance Tester is essentially a dial gauge mounted
on an extension tube which screws into the spark-plug holes and bears on the piston crown
when at top dead centre. A syringe is used to 'suck/blow' the piston up and down, the
difference in gauge readings being converted into combined bearing clearance. The
instructions recommend that this check be performed every 12.5 flight hours when the
spark-plugs are removed for inspection in accordance with the Rotax service schedule.
Maximum wear figures are provided with the instrument, but records should also be kept so
that any trend can be detected in advance. It is understood that the Popular Flying
Association acknowledges that this device has been effective in preventing failures of
this nature and has published an article entitled 'KNOW YOUR BIG END WEAR' in the December
'95/January '96 edition of its magazine, 'Popular Flying.'
At the time of the accident the engine had achieved 730 hours
since manufacturer. At 298 hours since manufacture the engine's crankshaft failed and the
engine was rebuilt by a manufacturer's approved agent. Between 5 and 6 hours before the
accident the pilot and an engineering friend had performed a check of the big end bearing
wear using the 'Cyclone Conrod Bearing Clearance Tester'. The rear cylinder bearing wear
was found to be 0.015 inch, which was within the limits specified.

THREADS, AS THEY NORMALLY DO (ACCORDING TO THE DEALER) FROM THE FACTORY.

ROTARY VALVE GEAR OIL SUMP FASTENER BROKE AND SUMP FELL TO A LOWER LEVEL THAN THE
CRANKSHAFT. LACK OF LUBRICATION FROM SUMP CAUSED BRASS ROTARY VALVE GEAR TO SHEAR OFF ITS
TEETH. RESULT WAS THE ROTARY VALVE SLIPPED AND BECAME OUT OF TIME WITH ENGINE; THUS,
ROUGHNESS AND POWER LOSS OCCURRED.

AFTER TOTAL ENGINE FAILURE WITH NO WARNING, ROTARY VALVE SHAFT WAS FOUND DEFECTIVE. ROTARY
VALVE SPLINES WERE FOUND MEL TED OFF. FAILURE WAS FOUND TO BE DUE TO ROTARY VALVE OIL
BEING DILUTED BY WATER. WATER WAS FOUND IN ROTARY VALVE OIL T ANK. SEAL HAD FAILED.
INSPECTED THREE OTHER AIRCRAFT AND FOUND SAME PROBLEM. SUBMITTER STATED THIS IS AN
EXPERIMENTAL AIRCRAFT, BUT A PROBLEM NEEDS TO BE ADDRESSED.
